Program overview
Following an individual鈥檚 discharge from participating hospitals, four follow-up contacts are offered over the next 90 days. Our mental health professionals will call, email, or text at different intervals:
- 1-2 days,
- 7-10 days,
- 30-60 days,
- and 60-90 days after discharge
These contacts are intended to provide support, connection to resources, risk assessment, safety planning, and, if necessary, crisis intervention. Further follow-up contacts may occur as needed or requested.
For youth patients, follow-up calls are made to the parent or guardian; however, if the child is older than 12 years old, the parent or guardian may choose to grant permission for us to speak with their child directly.
